What We Evaluate During Your Assessment

During your first visit, your certified Wound Care Nurse Practitioner will perform a comprehensive evaluation that covers:

  • Wound type, size, depth, and staging (using PUSH Tool or Wagner Scale as appropriate)

  • Wound bed condition — granulation tissue, slough, eschar, and necrotic tissue

  • Periwound skin integrity, edema, and signs of infection

  • Exudate amount, color, and odor

  • Presence of undermining, tunneling, or sinus tracts

  • Vascular status and perfusion (including ABI screening when indicated)

  • Pain level and patient-reported comfort

  • Nutritional status and contributing comorbidities (diabetes, PVD, venous insufficiency, lymphedema)

  • Current medications that may affect healing

  • Patient and caregiver knowledge and home environment

Your Personalized Wound Care Plan

Based on the assessment findings, we develop a written, evidence-based wound care plan tailored to you — not a generic protocol. Your plan will include the appropriate wound care products and dressings, a recommended visit frequency, measurable healing goals with a realistic timeline, coordination with your primary care physician or specialist, and education for you and your family on what to do between visits.

We document everything in detail and communicate findings to your referring provider or facility team so your entire care team is aligned from day one.

Who Benefits From a Wound Assessment?

Our assessments are appropriate for patients who have a wound that is not healing after 2–4 weeks of standard care, patients recently discharged from a hospital or SNF with an open wound, patients with diabetes, vascular disease, or immunocompromising conditions, residents of assisted living facilities, board-and-care homes, or group homes whose wounds are being managed with basic dressings, and any patient or family caregiver who wants a second opinion on a wound treatment plan.

How the Assessment Visit Works

  1. Schedule — Call or submit our online form. We'll confirm your insurance, schedule your visit, and send you a confirmation.

  2. We Come to You — Your NP arrives at your home, SNF, ALF, or care facility at the scheduled time with all necessary equipment.

  3. Full Evaluation — The assessment takes approximately 30–45 minutes. We examine the wound, review your history, and ask questions.

  4. Care Plan Delivered — Before we leave, we explain your care plan in plain language. A written summary is sent to your referring provider.

  5. Ongoing Care Begins — Follow-up visits are scheduled based on your wound's needs — typically 1–3 times per week in the acute phase.

Insurance Coverage

Wound assessments and ongoing wound care visits are covered under Medicare Part B for homebound patients and under Medicare Part A for residents of skilled nursing facilities. We also accept UnitedHealthcare and Aetna. Our team handles verification and paperwork so you can focus on healing.
