Every wound tells a story — about what caused it, what's keeping it from healing, and what it needs to close. Effective wound care starts with reading that story correctly. A thorough assessment is the foundation of every successful treatment plan, and skipping or rushing this step is one of the most common reasons wounds fail to heal.
At Emet Mobile Wound Care, every patient starts with a detailed in-home assessment by a certified wound care nurse practitioner. From there, we build a personalized plan tailored to the patient's wound, medical history, and goals.
Comprehensive Assessment
What a Comprehensive Assessment Includes
Our initial wound assessment is far more than a quick look at the wound. We evaluate:
- Wound assessment — size, depth, location, tissue, drainage, edges, and surrounding skin.
- Pain & symptoms — daily discomfort and dressing change pain.
- Medical conditions — diabetes, vascular disease, nutrition, and healing-impacting medications.
- Vascular status — circulation, pulses, skin color, and temperature.
- Risk factors — pressure, mobility, continence, nutrition, and home environment.
- Wound history — previous treatments and outcomes.
- Photo documentation — dated progress photos at each visit.
Building the Care Plan

01 — The clinical interventions the wound needs (debridement, dressings, advanced therapies).
02 — The visit schedule and expected duration of treatment.
03 — Specific instructions for caregivers between visits.
04 — Pressure offloading, nutritional, or activity recommendations.
05 — Coordination with the patient's primary care provider, home health agency, and any specialists.
06 — Clear goals and benchmarks for measuring progress.
Communication
Communication With Your Care Team
We keep the patient's other providers informed throughout the course of treatment. That means timely updates to primary care providers, clear documentation for home health agencies, and direct communication when something needs immediate attention. Patients shouldn't have to be the messenger between their providers — that's our job.
